Swift Charisma 545 2003 - wheel fell off

Just returned home after a lovely weekend away. Travelled on motorway for 1.5 hours as we turned on to our caravan storage site the off side caravan wheel came competely off including the hub ! We were very lucky for it to happen off the M1 and were only
travelling at low speed as my husband was turning the corner. Has anyone else had this problem ? Ive looked online and most problems seem to be not checking the wheel nuts but this was not the case with us. Fortunatley neither of us or my 5 year old were hurt.
The breakdown veichle was able to jack up the caravan for the wheel to be put back on and we very slowly towed it to our storage pitch.
