Germany low emissions disc

We have a low emissions disc for Germany on our car but have recently fitted a personalised number plate. Would be grateful to hear from anyone who knows if the disc we have will still be valid, (we could take proof that it is the same vehicle) or do we have to re-apply. I know this is easy but don't want to pay again if we don't have to.
