Jockey wheel dropping en route

Years ago, I had a jockey wheel drop on the caravan that we had then.  I have always really tightened the clamp to make sure that it has never happened again but it happened again yesterday.  What is the best step to take to ensure that the wheel does not
drop whilst travelling?
